Licensed Practical Nurse (LPN) – Long-Term Care (LTC) Location: Grand Junction, COAssignment Length: 8 WeeksShift: Variable Night ShiftsShift Length: 12 HoursGuaranteed Hours: 36 Hours/WeekCompensation Local Rate: Up to $40/hr (based on experience)Travel Rate: Up to $49/hr (based on experience)Position Overview "Navitas Healthcare, LLC" is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join a dedicated long-term care and skilled nursing team in Grand Junction, Colorado. The ideal candidate will possess strong clinical and patient care skills, with experience in long-term care, rehabilitation, or post-acute settings preferred.This position requires flexibility for night shifts and the ability to thrive in a fast-paced healthcare environment while delivering exceptional resident-centered care.Key Responsibilities Provide direct nursing care in accordance with individualized care plans and physician ordersAdminister medications and treatments safely and accuratelyMonitor, assess, and document resident conditions and report any significant changesPerform routine nursing procedures including wound care, vital signs, and basic assessmentsAssist residents with activities of daily living (ADLs) while ensuring dignity, comfort, and safetyCollaborate with nurses, providers, caregivers, and interdisciplinary teams to support quality patient outcomesMaintain accurate and timely clinical documentationAdhere to infection prevention, safety, and regulatory guidelinesSupport care planning initiatives and resident-focused treatment goalsRespond promptly and professionally to resident needs during assigned shiftsQualifications Active Licensed Practical Nurse (LPN) license in the state of Colorado requiredGraduate of an accredited Practical Nursing programPrevious experience in long-term care, skilled nursing, rehabilitation, or post-acute care preferredCurrent BLS/CPR certification preferredStrong clinical, assessment, and documentation skillsAbility to work independently and collaboratively within a healthcare teamExcellent communication and interpersonal skillsCompassionate, dependable, and patient-focused approach to careAbility to manage multiple patient assignments efficiently in a fast-paced environmentFor more details contact at hdavda@navitashealth.com or Call / Text at 516-862-1169.About Navitas Healthcare, LLC certified WBENC and one of the fastest-growing healthcare staffing firms in the US providing Medical, Clinical and Non-Clinical services to numerous hospitals.
