Physical Therapist-SS

Specific duties and responsibilities:Complete initial eligibility evaluations, added assessments, Individual Family Service Plans (IFSP), 6 month progress reviews, annual evaluations, exit evaluationsWrite Evaluation Summary Reports and contributing information on IFSP's and participate in all required meetings pertaining to position purposeProvide direct services utilizing the Family Training Model to promote appropriate child developmentInvolve parents/families in techniques and activities to facilitate appropriate child development, as per Family Training ModelFacilitate workshops, when appropriateComplete all required reports, session notes, and all other documentation applicable to dutyParticipate in staff meetings and/or professional developmentFulfill job-related responsibilities in a manner reflecting professional and ethical standards of conduct that apply to their specific profession or disciplinePerform other duties at Program Director's request
