Provides client assessments related to education.Responsibilities (Your Job):Under the direct supervision of a registered nurse:Handles and serves clients in a manner conducive to their safety and comfort and prioritizes based on the client's current care needs.Medication and treatment administration (including but not limited to: wound packing, irrigation, indwelling catheter care, etc), pain management and vitals.Coordinates schedules for clients requiring rehabilitation therapy.Follows infection control techniques in performing client care.Exchanges information necessary for quality patient/client care.Guides direct care staff in managing emergency care situations and following established emergency procedures and practices.Timely, accurately, and thoroughly prepares proper documentation including but not limited to electronic record keeping, in a manner that conforms to prescribed style and format as required by Federal and State regulations and Company policy.Proficiently and accurately monitors and reports client condition changes (signs & symptoms) to the Registered Nurse, attending physician, family, and interdisciplinary team members follows through on client care services needed to meet the individualized needs of each client.Completes initial and ongoing assessments by gathering data on time to promote an individualized plan of care for each client.RequirementsMust be currently licensed in the state of Texas as a Vocational NurseCPR/BLS CertificationPreferred experience as an LVN in an HCS Waiver ProgramSocial Security CardTexas Driver's License or State IDCriminal Background Check prior to hireNurse Misconduct Registry Check prior to hireMinimum of 1 year experience preferred but not required.Empathy for the needs of the clients.Treats clients, staff and the public with courtesy, respect and presents a positive public image.Ensures confidentiality and security of the client's medical information.Interpersonal skills, including exceptional patience, listening skills and ability to work both in team settings and independently.BenefitsCompetitive contract rate (negotiable based on experience and scope of services)Flexible scheduling to fit your availabilityProfessional autonomy in delivering assigned servicesOpportunities to work across multiple provider locationsCollaborative and supportive leadership teamMileage reimbursement for approved travel between sitesReferral program for introducing qualified professionalsExposure to multi-site programs and operational environmentsAccess to internal training and regulatory updates
