You can handle hard conversations, you document cleanly, and you don't let discharge planning drift.What You Will DoLead psychosocial assessments, care planning, and ongoing support for residents and familiesCoordinate safe, appropriate discharges (and document the "why" behind the plan)Manage grievances/complaints professionally and ensure follow-up is completed and trackedSupport residents' rights, advance directives, and decision-making resourcesCollaborate daily with Nursing, Admissions, Therapy, and the Business Office to remove barriersAssist with behavior support plans, mental health coordination, and community resource referralsMaintain accurate, survey-ready documentation that reflects resident needs, preferences, and interventionsParticipate in QAPI and support investigations as needed (abuse/neglect allegations, elopement risk, etc.)What Success Looks Like HereDischarges are planned, safe, and documented —not rushed, messy, or last-minuteFamilies feel informed and respected (even when they don't like the answer)Concerns don't sit—grievances are owned, addressed, and closed outSurveyors can open your documentation and clearly see: assessment → plan → action → follow-upMust-HavesBachelor's degree in Social Work or related field required (MSW preferred)Skilled Nursing Facility experience strongly preferredStrong written documentation skills and comfort with difficult conversationsOrganized, consistent follow-through, and able to work fast without losing accuracyOur StandardAt The Overlook at Rowan, we live by grit with grace, straight-shooter communication, and resident-first accountability.
