Certification at Paradigm doesn’t just meet a requirement; it unlocks access to higher‑acuity work.About the Role - This is a field / home‑based field position with travel (up to 2 hours one‑way) to physicians’ offices, hospitals, and other care locations.As a Nurse Case Manager at Paradigm, you will be responsible for the medical case management of work‑related injuries, including:Comprehensive assessment, planning, coordination, implementation, and evaluation of careCollaboration with injured workers, insurance carriers, medical providers, employers, and attorneysMonitoring clinical progress and outcomes and reporting to stakeholdersManaging complex and catastrophic cases aligned with your expertiseAt Paradigm, people come first — and the work you do every day directly improves lives.Qualifications:Required: Current Registered Nurse (RN) license in GeorgiaNationally recognized case management certification:CCM, CRRN, COHN, or CDMS(This advanced credential reflects your expertise and positions you to manage higher‑acuity, premium‑pay cases.)GA Rehabilitation Supplier registrationValid driver’s license with good driving historyPreferred: Experience in workers’ compensation case managementSkills for Success :Strong organization and time‑management skillsExcellent written, verbal, and interpersonal communicationComfort working independently with minimal supervisionProficiency with technology and documentation systems
