Position OverviewThe Home Care Occupational Therapist provides skilled occupational therapy services directly in patients' homes. The role is responsible for evaluating functional abilities, identifying barriers to independence, and implementing treatment plans that support safe and effective performance of daily living activities in a home environment.Core ResponsibilitiesConduct in-home occupational therapy evaluations and establish patient-centered goalsDevelop and implement individualized treatment plans to improve functional independenceProvide therapy interventions targeting activities of daily living (ADLs) and instrumental activities of daily living (IADLs)Assess home safety and recommend modifications to reduce risk and improve accessibilityTrain patients and caregivers on adaptive strategies, compensatory techniques, and safe mobility practicesRecommend and instruct on use of adaptive equipment and assistive devicesDocument evaluations, progress notes, and discharge summaries in compliance with regulatory and agency standardsCoordinate care with physicians, nurses, physical therapists, and other healthcare professionalsMonitor patient progress and adjust interventions as clinically indicatedEducate patients and families on injury prevention, fall prevention, and condition managementMaintain compliance with Medicare guidelines, state regulations, and agency policies QualificationsDegree in Occupational Therapy from an accredited program (BS, MS, or OTD)Current Occupational Therapist license in the practicing stateNBCOT certification (if required by state)Valid driver's license and reliable transportationAbility to travel regularly within assigned geographic area Preferred ExperiencePrior experience in home health, outpatient, or rehabilitation settingsExperience with geriatric, neurological, orthopedic, or medically complex populationsFamiliarity with electronic medical record (EMR) systemsStrong independent clinical judgment and time management skills Skills and AbilitiesAbility to work independently in a field-based environmentStrong communication and patient education skillsSound clinical reasoning and problem-solving abilityAdaptability to varied home environments and patient needsPhysical ability to safely assist patients with functional mobility tasks Work EnvironmentField-based position requiring travel to patient residencesFlexible scheduling based on caseload and patient needsWork performed in a variety of home settings and conditionsMay require physical activity including standing, bending, and assisting patients
