We are seeking a compassionate and dedicated Care Manager(RN) join our team at Pediatric Home Service (PHS). The purpose of this role is to provide comprehensive care coordination for our pediatric clients and their families, ensuring positive outcomes, optimal healthcare utilization, and seamless communication between healthcare providers and community stakeholdersPosition: Care ManagerLocation: Fully RemoteHours: The hours are full-time (8a-5p) with some flexibility in scheduling.Job DetailsAt Pediatric Home Service, we’re on a mission to take care of the child—and we’re looking for a Care Manager (RN) who is passionate about coordinating care, advocating for families, and improving outcomes for children with complex medical needs.In this role, you’ll partner with families, providers, and community resources to ensure every child receives the right care at the right time.Coordinate comprehensive care plans for pediatric patients with complex needsCollaborate with families, providers, and community partners to support positive outcomesEvaluate patient needs and recommend services, therapies, or equipmentParticipate in care conferences and ensure alignment with care plansServe as a key resource for clinical teams and families navigating careTrack outcomes and support data-driven care decisionsHelp improve processes, programs, and patient experienceQualificationsActive Registered Nurse (RN) license (required)Bachelor’s degree or equivalent experienceExperience in care coordination, case management, or home healthStrong communication and collaboration skillsAbility to manage multiple priorities and think criticallyPassion for patient advocacy and improving healthcare outcomesBackground in pediatrics, home care, or complex careExperience with EHR systems and care coordination toolsSocial work background or strong community resource knowledgeThe CompanyPediatric Home Service is a pediatric home health agency that has provided high quality DME and nursing services to children that are medically fragile since 1990. The majority of the clients we serve are age’s birth through 21 years old and have a variety of technologies that might include a trach, ventilator or g-tube.Salary RangeAt PHS, we are committed to providing specialized care for medically complex children. We are dedicated to offering fair and transparent compensation to our team members.
