RN Clinical Coordinator- Home Infusion

Registered Nurses!! Come join our growing team! We are seeking a highly motivated nurse with excellent organizational and communication skills to facilitate the coordination of patient and nursing services. This position offers the best of a Registered Nurse position including clinical assessment, patient education and screening, communicating between departments, patients, staff, referral sources and developing relationships with patients and staff. Your Role: Streamline Care: Receive and manage patient referrals, physician orders, and documentation to ensure smooth and effective care delivery. Collaborate & Coordinate: Work closely with physicians, in-office infusion Pharmacists, Registered Nurses, and billing staff to provide seamless patient care. Ensure Quality: Review referrals for appropriateness, clarify orders, and maintain the highest standards of care while adhering to policies and regulatory requirements. Communicate & Educate: Handle insurance information, data entry, patient care scheduling, and provide vital teaching and counseling to patients and their families. Come check us out! Enjoy the hours of Monday- Friday, 8:30-5:00pm with some on-call rotation, take part in our Performance Improvement program while providing support and direction for Registered Nurses in the field.
