Within this role, you will receive referrals for individuals from at-risk populations from interdisciplinary team member including physicians, Case Managers, staff nurses and other members of the care team, and will be assigned to a team to follow through acute care continuum. Healthcare related degree preferredMinimum three (3) years in social work, management of resources, or medical field.
