Pharmacy Care Coordinator
Schedule & Work EnvironmentHours of Operation: 8:00 AM - 6:30 PM Monday-FridayLocation: Onsite - River North, ChicagoKey ResponsibilitiesServe as a primary point of contact for patients, healthcare providers, and insurance companiesHelp patients navigate the specialty pharmacy process and understand steps required to access medicationsRespond to patient inquiries with professionalism, empathy, and accuracyPerform insurance verification, prior authorizations, copay assistance, and financial assistance coordinationEnter and maintain accurate prescription and patient data in pharmacy management systemsRequest new prescriptions and obtain clarifications from prescribers under pharmacist supervisionCoordinate with partner pharmacies, providers, and internal teams to ensure timely medication accessMaintain detailed and compliant documentation of patient interactions and case statusEnsure HIPAA compliance and patient confidentiality at all timesProvide technical support related to pharmacy systems or patient portalsParticipate in team meetings and contribute ideas for process improvementSupport additional projects and initiatives as assignedWe will consider for employment all qualified Applicants, including those with criminal histories, in a manner consistent with the requirements of applicable federal, state, and local laws, including the City of Los Angeles' Fair Chance Initiative for Hiring Ordinance (FCIHO), Los Angeles Fair Chance Ordinance for Employers (ULAC), The San Francisco Fair Chance Ordinance (FCO), and the California Fair Chance Act (CFCA).
