Job Title: Community Outreach CoordinatorReports to: Director of Community OutreachBASIC PURPOSEThe Community Outreach Coordinator facilitates and organizes waiver applications for qualifiedmembers of the community. The Community Outreach Coordinator develops relationships withprospects and referral sources to assist with the Home and Community Based Servicesapplication process and identifies additional business opportunities.Community Outreach Coordinators are responsible for working & communicating with the PAIndependent Enrollment Broker (PAIEB), Area Agencies on Aging (AAA), Financialinstitutions, and County Assistance Offices (CAO) to ensure all waiver applications are beingprocessed in a timely manner.Community Outreach Coordinators are expected to establish and maintain professionalrelationships with all referral sources, including but not limited to the following: Hospitals,independent living facilities, assisted living facilities, sub-acute rehabilitation facilities, SeniorCenters, Senior housing facilities, County Assistance Offices, hospice organizations, servicecoordination companies, area agencies on aging, and physicians.PRIMARY DUTIES AND RESPONSIBILITIES● Responsible for scheduling assessments for prospects with the PAIEB & Area Agencieson Aging● Conducting weekly follow up calls and maintaining strong relationships with prospectson your case load● Collecting and submitting required documentation to the PAIEB and CAO’s● Develop and execute a marketing plan to meet or exceed monthly, quarterly, and annualgrowth targets.● Adheres to and promotes company policies and procedures● Builds and cultivates relationships with industry contacts to gain industry knowledge,referrals, and business development leads● Ensures confidentiality of all consumer records and personnel files in accordance withagency and HIPAA guidelines● Ensures all services, functions, and employee practices are in accordance withAffirmative Action policies and laws● Responsible for ongoing compliance with all current federal, state, and local regulations,company policies and procedures, and reimbursement guidelines● Demonstrates a commitment to advancing 365 Health Services, LLC● Must possess the personal characteristics of professionalism and commitment to highstandards with a strong work ethic, confidence, creativity, innovation, integrity, andstability● Performs other duties as necessaryMINIMUM REQUIREMENTS● Bachelor’s Degree (preferred)● Must meet all federal, state, and local requirements● Excellent written and verbal communication skills● Strong analytical skills● Results driven, sense of urgency, and high standard of professionalism
