P Job Position: RN II-Case Management/ Utilization Review Department: RC - Revenue Cycle Location: San Luis Obispo, CA 93405 Duration: 13 weeks with high possibilities of extension Shift: 4 x 10s (7:00AM - 05:30PM) Weekly Range $3000-$3200 Responsibilities for this position include, but are not limited to: Job Summary The RN II - Case Management/Utilization Review (CNA) is responsible for coordinating patient care, conducting utilization reviews, and supporting discharge planning processes in an acute care hospital setting. This role ensures appropriate resource utilization, compliance with healthcare regulations, and optimal patient outcomes. Key Responsibilities Perform utilization review to ensure medical necessity and appropriate level of care Coordinate and manage patient care plans throughout hospitalization Collaborate with interdisciplinary teams to support timely and effective discharge planning Monitor patient progress and adjust care plans as needed Ensure compliance with hospital policies, regulatory standards, and payer requirements Advocate for patients to ensure quality care and efficient resource use Minimum Qualifications Current Registered Nurse (RN) license in the state of practice (Required) Basic Life Support (BLS) certification through the American Heart Association (Required) Associate Degree in Nursing (ADN) or equivalent (Required) Minimum of 1 year of hospital-based care management experience (Required) 3-5 years of nursing experience in an acute care hospital setting (Required) Preferred Qualifications Bachelor's Degree in Nursing (BSN) Recent experience in utilization review, discharge planning, and/or inpatient case management Tryfacta is an Equal Opportunity/Affirmative Action Employer.
