The registered nurse plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities. Responsibilities: Provides emergency, PRN, routine, and admission visits as necessary or scheduled. Assumes primary responsibility for hospice patients during on-call hours. Provides ongoing assessments of physical, psychological, and social and spiritual needs of the hospice patient and family and initiates appropriate interventions. Assesses and evaluates patient's status by following the plan of care, regularly reevaluating patient, and family/caregiver needs, participating in revising the plan of care as necessary based on patient status and needs. Develops a care plan that establishes goals, based on nursing diagnosis, and incorporates palliative nursing interventions/actions. Includes the patient and the family in the planning process. Qualifications: Hold a current unencumbered license as a RN or LPN. Hospice health experience preferred but not required. Must be computer proficient in typing and various programs, including background in EMR. Possess and maintains current CPR certification. Must be flexible to adjust to changing tasks frequently. Must have a valid driver’s license, reliable vehicle, maintain valid auto liability insurance, and be able to drive throughout service area under a variety of weather conditions. Excellent verbal and written communication skills. Work Location: In person
