ACCS is funded through theDepartment of Mental Health and is a multidisciplinary outreach approachfor persons served in need of psychiatric and other services livingindependently in the community. This involves active engagementincluding assertive outreach, consistent assessment and treatmentplanning, skill building, symptom management, addiction treatment, andbehavioral and physical health monitoring and support. YOUR ROLE AS A REGISTEREDNURSE: ACCS teams include licensed clinicians, RNs, housing specialists,substance use specialists, direct care staff, and peer/familyspecialists. The Team works collaboratively to provide person-centered,evidence-based services in support of Persons\' treatment goals. The RNcoordinates those functions and activities that provide for, maintainand promote the health of Persons served. The RN provides direct supportto Persons in order to assess needs, participates in medical reviews,and provides counseling in areas of health and psycho-physiologicalwell-being, and liaisons with other medical and community-basedproviders. This is an administrate RN role. REQUIRMENTS: BachelorsPreferred MA Registered Nurse License Required Bilingual(English/Spanish) candidates required Two years of experience workingwithin mental health and/or substance use field is preferred.
