OJJDP Violence Prevention Specialist/Care CoordinatorThe Violence Prevention Specialist/Care Coordinator (Nurse, MSW, LSW, LCSW) will serve several roles for the Comprehensive Hospital-Based Adolescent Violence Assessment and Treatment at Children's Hospital Colorado (CHAVAT@CHCO). This is a full-time (1.0 FTE) grant-funded position.Key Responsibilities:Coordinate patients interfacing with the CHAVAT, including reviewing charts for eligibility for the program, providing outreach to families, and coordinating intakes.Conduct assessments and provide brief motivational interviewing interventions for youth at risk for or having already perpetrating violence.Conduct MI-focused interventions focusing on ways to reduce both violence perpetration, gun carrying, and gun violence in the outpatient setting.Communicate treatment plans to the other behavioral health specialists involved in the CHAVAT as well as coordinate appointments with youth and their families.Coordinate wrap-around services, setting up appointments between youth identified in the emergency department or hospital setting for outpatient management.Participate in the care of approximately 100 clients annually.
