Registered Nurse - Functional & Lifestyle Medicine Emphasis

Job DescriptionThis RN role is crucial to patient care that combines traditional primary care with functional and lifestyle medicine principles, and holistic approaches to guide patients in achieving optimal wellness. The RN is also an educator that provides compassionate, culturally sensitive care while respecting patient learning preferences. The RN supports the team of providers, coordinating the delivery of integrative health strategies and empowering patients to be proactive in preventing disease to support their longevity.ResponsibilitiesClinical Care & Patient SupportPerform comprehensive patient assessments, including vital biometrics, detailed health histories, symptoms/chief concern, and lifestyle review.Maintain accurate up to date patient electronic medical records and review care plans incorporating conventional, functional and lifestyle medicine principles.Educate patients on health promotion and disease prevention while implementing lifestyle modifications, nutrition, and functional and lifestyle medicine principles.Support chronic disease management and assist with typical and specialty lab testing, including hormone panels, microbiome analysis, and metabolic assessments.Participate in ongoing training and collaborate with the care team to integrate conventional, functional, holistic and behavioral treatment strategies.Nurse Coaching & Patient EmpowermentUse motivational interviewing, behavioral change techniques, and coaching skills to guide patient goal setting and accountability to sustainable health improvements.Educate patients on the functional medicine approach, mind-body medicine, lifestyle changes, nutrition, supplements, exercise, stress management and mindfulness techniques as appropriate.Empower patients to actively engage in their health and self-care practices through a healthy lifestyle which may include functional and lifestyle medicine principles.Coordination & Patient AdvocacyBuild strong, long-term relationships with patients and families, focusing on preventive and proactive care.Act as a liaison between patients and providers, ensuring smooth communication and follow-up, and contribute to case conferences and patient-centered care planning.Skills & QualificationsEducation: RN/BSN required; Functional Medicine Certification (FM) preferred.Experience: Minimum 4 years of experience in healthcare, ideally in family, functional, or lifestyle medicine.Licensing: Must be licensed in Wisconsin.Certifications: BLS required; health coaching or behavioral change coaching certification desirable.Skills: Strong communication, EMR experience (Athena preferred), organizational, critical thinking, and decision-making skills. Experience in patient education, chronic disease management, and lifestyle medicine approach required.Nursing Technical Skill: IV therapy and phlebotomy experience and proficiency desired, or willingness to learn and maintain proficiency.Physical RequirementsAbility to stand/walk for extended periods, perform nursing procedures, and lift up to 50 lbs.Willingness to attend evening or off-hour events, including community educational offerings and group/shared medical visits.Background Check RequirementsAll candidates for employment will be subject to pre-employment background screening for this position, which may include Nursys verification, motor vehicles, drug testing and credit checks based upon the position description and job requirements.
