This is a full-time, direct hire opportunity with a supportive care team and a mission-driven organization focused on helping participants remain safe, independent, and healthy in their homes.The RN delivers skilled nursing services, coordinates care, and supports participants and families through education, assessments, and clinical oversight.Key ResponsibilitiesProvide skilled nursing care to participants in their homesPerform assessments, re-assessments, and ongoing evaluations of participant needsImplement physician orders and coordinate plans of careAdminister medications and treatments; monitor medication complianceEducate participants and families on medications, treatments, and health managementObserve, document, and report changes in participant conditionComplete clinical documentation, progress notes, care conference summaries, and discharge paperworkTriage participant and staff phone calls and prioritize care needsCoordinate specialist appointments, outside services, and hospital admissions as neededDetermine appropriate level of care and collaborate with home care resourcesProvide preventive and rehabilitative nursing interventionsParticipate in care conferences, peer review, quality improvement, and utilization reviewSupervise LPNs and HHAs/CNAs in accordance with regulatory requirementsRefer participants to PT, OT, SLP, and Medical Social Work services as appropriateEnsure quality care delivery through chart reviews, participant feedback, and care plan monitoringPerform additional duties as assignedRequired QualificationsActive Colorado RN licenseCurrent First Aid and BLS certification (AHA or Red Cross)Valid driver's license, reliable transportation, and auto insuranceMinimum 1 year of experience working with frail or elderly populationsPreferred QualificationsBachelor's degree in Nursing (BSN)Gerontological Nursing certification3+ years of healthcare experience with a focus on geriatricsEpic EMR Experience a plus
