Job OverviewEbenezer Integrated Care & Rehab (EICR) serves seniors with long-term, transitional, and dementia care needs in the heart of downtown St. Paul. As part of Fairview's Community Health and Wellness Hub, EICR is committed to delivering innovative, collaborative care. The Fairview Community Health and Wellness Hub opened in 2022 to deliver a variety of services to the community-from primary and mental health care to enrichment options for seniors, food access programs, and community gathering spaces. The Hub brings Fairview services together with local organizations-including Minnesota Community Care and Sanneh Foundation-to make it easier for people to access what they need to be well.We are seeking a Nursing Assistant (CNA/NAR) for our newest Skilled Nursing Community (Transitional Care units, Long Term Care) in downtown St. Paul, Minnesota! This role is responsible for providing resident care and services.This CNA/NAR schedule includes;40 hours every two weeks ; Part TimeEvening Shift ; 2:00pm-10:30pmBlock Schedule:Week 1: Thursday, Saturday, and SundayWeek 2: Wednesday and Friday* Long Term Care Unit* Every other weekend and holiday rotationResponsibilitiesAssists residents with personal cares based on individual plan of care:Provides AM/PM caresProvides full bath/shower weeklyProvides oral hygiene per care planProvides nail care, shaving as appropriateChecks hearing aids; cleans eyeglasses dailyAssures that residents are clean, dry, and dressed appropriatelyProvides offloading for residentsPerforms Restorative Nursing Programs per facility policy.Provides adequate nutrition & hydration (distributes meal trays, assists residents as needed, documents intake/output meal consumption, provides nourishment & water between meals as ordered)Assures individual toileting plan is followedAnswers call lights promptlyObserves residents and reports status to appropriate staff:Receives report at the start of the shiftGives report to team leader at the end of the shift & PRNNotifies appropriate staff of any changes in a timely & appropriate manner (i.e. incidents, skin integrity, behavior changes, illness)Offers pertinent observations & input for documentation and care plan updatesUtilizes Nursing Assistant Kardex or Care sheetTakes and documents weights as orderedDocuments resident cares promptly in EHRResponds to emergency situations appropriatelyHandles resident and family complaints tactfully and refers concerns to appropriate staff membersFollows Ebenezer policies and proceduresReports resident change of condition to Licensed NurseAttends mandatory, annual, and any other educational opportunities on site.Performs environmental and other responsibilities as assigned:Maintains order & cleanliness in resident roomsMaintains order & cleanliness in other work areasReports malfunctioning equipment to Maintenance DepartmentAssists with admissions, transfers & discharges as neededAnswers phones with proper customer service etiquettePerforms other duties as assigned and directedRequired Qualifications* less than 1 year* Nursing Assistant Registered Registered (NAR) on the MDH Nursing Assistant Registry by start date - OR- Individual who is hired into Ebenezer's Nursing Assistant Course or Ebenezer's Pre-CNA job for Nursing Assistants; will be granted a 4-month post hire to become active on the Minnesota Nursing Assistant Registry.)Preferred Qualifications* High School Diploma or GED* 1 yearBenefit OverviewEbenezer offers a generous benefits package, including but not limited to medical, dental, vision plans, life insurance, short-term and long-term disability insurance, PTO and Sick and Safe Time, tuition reimbursement, retirement, early access to earned wages, and more!
