You'll work closely with RNs and physicians to ensure continuity of care and comfort for our patients.Key Responsibilities:Follow and implement the patient's plan of care as directed by the RNProvide timely and accurate documentationConduct assessments and report findings to the care teamCollaborate with interdisciplinary team membersUse medical equipment and supplies efficientlyEngage in ongoing professional developmentWhat We're Looking ForValid California LVN licenseMinimum 2 years of direct patient care experienceAt least 1 year of hospice experience preferredCurrent CPR certificationReliable transportation with valid driver's license and insuranceWhy Bella Terra?We believe in empowering our team to make a difference.
