We are seeking a Travel Physical Therapist (PT) to join our home health team supporting patients in the Hot Springs and Malvern areas. This role is responsible for providing skilled physical therapy services, developing individualized care plans, and ensuring high-quality, patient-centered outcomes in a home-based setting.The ideal candidate is independent, clinically strong, and experienced in home health with a focus on OASIS documentation and care coordination.This Job Will Have The Following ResponsibilitiesPatient Evaluation & Care PlanningEvaluate patient functional status, physical therapy needs, and all body systems required for CMS documentationCollaborate with physicians to develop individualized plans of careComplete all patient evaluations and establish care plans within state-specific guidelinesReport evaluation outcomes, goals, and frequency of care within 24 hoursTreatment & Clinical CareProvide therapy services in accordance with physician orders and care plansTreat patients to relieve pain, restore function, and maintain optimal mobility using evidence-based techniquesMonitor patient progress and adjust treatments as neededObserve and report changes in patient condition to physicians or nurse supervisorsPatient & Caregiver EducationEducate patients, families, and caregivers on therapy plans, goals, and functional limitationsProvide guidance on exercises, mobility techniques, and overall health managementDocumentation & ComplianceComplete accurate and timely OASIS and visit documentation per agency standardsMaintain compliance with CMS requirements, policies, and clinical protocolsPrepare progress notes for each patient visit and submit within required timelinesProvide discharge summaries or status reports at least every 60 daysCollaboration & LeadershipCollaborate with interdisciplinary care teams to ensure coordinated patient careParticipate in staff meetings and case conferencesSupervise Physical Therapist Assistants (PTAs) and home health aides as applicableProvide in-service education and mentorship as requestedOperational & Field ResponsibilitiesTravel to patient homes across the assigned coverage area and nearby regions as neededMaintain schedule flexibility to meet patient care needs, including potential weekend rotations/holiday coverageAdhere to all safety, infection control, and therapy practice standardsQualifications & Requirements:Active and current Physical Therapist (PT) licenseHome Health experience requiredOASIS documentation experience requiredCurrent CPR certificationStrong clinical, documentation, and communication skillsBy providing your phone number, you consent to: (1) receive automated text messages and calls from the Judge Group, Inc. and its affiliates (collectively "Judge") to such phone number regarding job opportunities, your job application, and for other related purposes.
