Rehabilitation Case Manager

Job Description: Workforce Partner Navigator (WPN) Objectives: Increase Service Access: Enhance the number of individuals with disabilities served by leveraging resources across partner programs. Identify and address gaps in service access. Support Navigation: Facilitate successful transitions for youth and adults from high school to postsecondary training with clear vocational goals. Provide wrap-around supports to promote program completion and access to meaningful work-based learning activities. Embedded Collaboration: WPNs will be integrated within Work One centers and VR offices, acting as knowledgeable resources on services and programming. They will build relationships to support service delivery and streamline cross-referral processes for individuals with disabilities. Key Responsibilities: Connect individuals with disabilities to appropriate partner programs. Promote and guide braided service delivery across programs. Attend partner staff meetings and participate in cross-training initiatives. Serve as a resource for colleagues, fostering collaboration and knowledge sharing. Requirements: Education & Experience: Bachelor’s degree plus at least two years in training, social services, workforce services, vocational rehabilitation, or education. Knowledge: Familiarity with the Workforce Innovation and Opportunity Act, Rehabilitation Act, and Americans with Disabilities Act. Skills: Excellent verbal and written communication. Proficient in Microsoft Office Suite. Ability to work independently as a self-starter. Preferred Experience: Proven ability to build and maintain partner and stakeholder relationships. Experience working with individuals and job seekers with disabilities. Working Conditions: WPNs will operate across various Work One centers and VR offices within assigned regions, maintaining a regular presence at partner organizations. In-person meetings will be prioritized, with some remote work as necessary. Job Type: Contract Pay: $25.00 - $27.00 per hour Expected hours: 40 per week Schedule: Monday to Friday Work Location: In person
