Title: Physical TherapistLocation: Westbury New York USA 11590Rate:$45/hr on W2 all-inclusive without benefitsEssential Functions:1. Performs clinical assessment to identify patient needs and appropriateness of patient.2. Document, prepare/review and complete a Plan of Care (POC), progress notes, phone orders and other clinical record documentation on an ongoing basis. Revise the POC based on evaluation, ongoing assessment data. Works in consultation with the physician and as part of an interdisciplinary team.3. Plan and provide appropriate treatment for neuromuscular, neurological and orthopedic disorders based on medical referral and therapy assessments.4. Assist/instruct patient/family in transfer techniques appropriate to patient abilities. Instruct on use of equipment such as walkers, wheelchair, canes, braces, prosthetic devises and appliances in a safe manner. Utilize appropriate therapy equipment specific to patient diagnosis and needs. Orders appropriate rehabilitative equipment to meet patients' needs and adapts equipment, appliances to meet home limitations/requirements.SkillsCurrent State License as a Physical Therapist - RequiredProof of current CPR certification - RequiredCurrent driver's license.
