Maintains open communications with all members of the interdisciplinary healthcare team. Ensures Interim baseline care plans are within 24 hours of admissions. Rounds with consulting physicians (cardiac, wound care, etc.).Serves as a clinical coordinator, providing clinical guidance and hands on care as indicated, according to best practice guidelines and facility standards of care.Functions as clinical team leader. Directs, instructs, supports, and supervises ancillary personnel to assure that the needs of the primary assignment and patient's care plan are met.Assures and maintains a safe and infection free environment.Establishes/expands rehabilitative nursing skills on an ongoing basis.
