The Physical Therapist will evaluate, plan, and implement individualized treatment plans for patients to support rehabilitation and recovery. Responsibilities include assessing patient needs, developing therapeutic strategies, providing hands-on physical therapy treatments, and collaborating with patients, families, and other medical professionals. Additionally, the role involves monitoring patient progress, documenting treatments, and educating patients on proper techniques for ongoing care.QualificationsStrong expertise in Physical Therapy, Rehabilitation, and Patient CareKnowledge of Medicine and its integration into physical therapy practicesExperience with Home Care and providing personalized treatment plansExcellent communication and interpersonal skills to effectively engage with patients, families, and healthcare teamsLicensure as a Physical Therapist in the state of CaliforniaBachelor’s or Master’s degree in Physical Therapy or a related field (Doctorate in Physical Therapy preferred)Commitment to providing compassionate and patient-focused careAbility to work effectively in an on-site multidisciplinary setting
