This individual will perform assessments of patients in hospital and help with placement of patients into small homes, and as well as oversee a cohort of patients living in small residential homes to assure their clinical healthcare and non-clinical needs are met.This hybrid role is located in Green Bay, Wisconsin and surrounding areas. There may be occasional travel across the state. Most in person work will be local, and there will be a component of work from home as well.QualificationsThe individual can be either a nurse, physiotherapist, or occupational therapist. There may be opportunities for a related social worker role.Experience in the healthcare or caregiving industry. Ideally some inpatient hospital experience.Excellent bedside patient communication, interpersonal, and leadership skills.Bachelor's or Master's degree in Nursing (or OT, PT, or Social work potentially)Knowledge of Medicaid and Medicare and various plansKnowledge of Support and Social Services in the Green Bay region is helpfulTechnology skills are an asset
