Home Health Registered Nurse (RN)

Location: Denison, TX Employment Type: Full-Time About the Role Texas-Nurse.com is now hiring a skilled and compassionate Home Health Registered Nurse (RN). This role provides in-home skilled nursing care, patient education, and care coordination to help patients recover, manage chronic conditions, and remain safely at home. Key Responsibilities Perform comprehensive in-home nursing assessments and develop individualized plans of care Provide skilled nursing services including wound care, medication management, injections, and disease management Educate patients and caregivers on diagnoses, medications, and self-care Monitor patient progress and report changes in condition to physicians and the care team Coordinate care with physicians, therapists, and other healthcare professionals Complete OASIS assessments and timely clinical documentation Participate in care conferences and quality improvement activities Ensure compliance with Medicare, home health regulations, and agency policies Qualifications Current Texas RN license in good standing Home health experience and OASIS knowledge preferred Strong clinical assessment and critical thinking skills Ability to work independently and manage a field-based caseload Excellent communication and patient education skills Valid driver’s license and reliable transportation Why This Opportunity?
