Travel, Social Worker - LCSWLocation: Camden, MEShift: 5x8 Days, 08 am to 4 pmDuration: 14 WeeksGuaranteed Hours: 40Hourly Pay: $44/hrKey ResponsibilitiesConduct initial and ongoing psychosocial assessments to identify residents' emotional, social, and environmental needsCollaborate with interdisciplinary care teams to develop and implement individualized care plansCoordinate complex discharge planning, including community referrals, home health services, and durable medical equipmentProvide short-term counseling and crisis intervention to residents and familiesAdvocate for resident rights and assist with advance directives and power of attorney mattersSupport residents and families with financial resource guidance, including Medicaid and insurance navigationMaintain accurate, timely documentation in compliance with state and federal regulations Requirements:Licensure: Active Social Work license in the State of Maine (LSW, LMSW, or LCSW required)Education: Bachelor's degree in Social Work (BSW) required; Master's degree (MSW) preferredExperience: 1-2 years of recent healthcare social work experience preferred (SNF, Long-Term Care, or Acute Care)
