Physicians provide a comprehensive range of primary and specialty care services at the 333-bed hospital and outpatient facilities both on and off of the Eskenazi Health downtown campus including at a network of Eskenazi Health Center sites located throughout Indianapolis.FLSA StatusNon-ExemptJob Role SummaryThe Community Health Worker will work with patients at the health center and community level to enhance understanding and adherence to treatment protocols, identify social determinants of health, and help patients to set and achieve their self-management goals. The CHW will document home social and environmental assessments timely, accurately and with good quality, identifying barriers to accessing appropriate healthcare or other concerns within the patient's home environment.Essential Functions and ResponsibilitiesProactively contributes to Eskenazi Health's mission: Advocate, Care, Teach and Serve with special emphasis on the vulnerable population of Marion County; models Eskenazi Health's valuesDocuments health, social and environmental assessments timely, accurately and with good quality, identifying barriers to accessing appropriate healthcare or other concerns within the patient's home environmentPerforms home visits to patients to provide support, encouragement and guidanceDevelops supportive and therapeutic relationships with patients and their familiesAccurately documents home/telephone visit outcomes in a timely mannerUtilizes motivational interviewing to encourage patients to modify their behaviorsRecommends community resources for patients order to meet the basic five needs of : housing, food, clothing, transportation and utilitiesCarries a caseload of at least 100 patientsAdvocates for patients to ensure access to needed services which includes developing community relationships and resourcesIdentifies any high risk patient symptoms or circumstances that would result in risk for self-harm or harm to others, and notifies appropriate higher level clinical staff for consultationCompletes appropriate home assessment documentation, identifies barriers and refers patient to the appropriate community resourcesWorks with family, significant others and community supports to assist patients in meeting goalsAlters activities/behaviors to reflect and ensure adequate care appropriate to the age of patients servedProvides quality case management and provides training to patients on activities related to daily living skillsMonitors patient progress and reflects this in documentation during follow up visitsDemonstrates commitment to professional growth by actively participating in clinical supervisionJob RequirementsHigh school diploma or equivalentOne year of job-related work experience, performing similar functions in a similar roleValid Indiana driver's license that meets Eskenazi Health's driving policy requirementsKnowledge of the Marion County community and its residents having either lived in or around targeted areas; knowledge of available community resources and required application processesKnowledge, Skills & AbilitiesAbility to relate to a variety of people within different cultures, socioeconomic backgrounds and education levelsAbility to establish and maintain effective working relationships with superiors, coworkers, patients and familiesWillingness to work with patients and their family members or other support persons in the clinic, hospital, home, and community settingsBasic computer skillsAbility to work independentlyAbility to work effectively in a sometimes stressful and unstructured environmentTeaching/coaching skillsStrong interpersonal skills; ability to be both clear and concise in written and verbal communicationUse of discretion and independent decision making regarding patient safety and careWillingness and ability to carry out directives from other team membersAbility to use communication skills appropriate to the age of the patientAbility to interpret and apply complex directives/information/regulationsCrisis intervention skills helpfulAccredited by The Joint Commission and named as one of Indiana's best employers by Forbes magazine for two consecutive years and the top hospital in the state for community benefit by the Lown Institute, Eskenazi Health's programs have received national recognition while also offering new health care opportunities to the local community.
