Cardiovascular Heart Clinic RnFairview has an immediate opening for a cardiovascular heart clinic rn at the heart clinic located in maplewood, mn.
this is a 1.0 fte (80 hours per two week pay period), day shift, benefits eligible. we believe this schedule will be 8 hours, working monday through friday between 8:00 a.m. to 5:00 p.m.
provides nursing care to patients with cardiovascular disease within the scope of practice for a registered nurse. care is delivered in collaboration with the physicians and healthcare team. evaluates patients, reviews diagnostic tests and initiates/modifies diet, lifestyle and other therapies using nursing processes.
responsibilities
consistent, ongoing monitoring, assessment, documentation, and personalized education of patients with attention to detail and focus on the patient experience.
coordination of care with a variety of disciplines and facilities, to maximize patient outcomes and minimize recurrent hospitalizations.
case management-style nursing model with goal-directed care consistent with high quality care.
facilitate patient assistance medication programs or grants available to those meeting guidelines advocate/assist in referral process for appropriate therapies.
required qualifications
associate of science in nursing
2 years of post-coronary or coronary critical care experience as a registered nurse.
knowledge of adult cardiovascular health and disease processes as well as related diagnostic and treatment modalities.
ability to perform a brief history and physical examination and to recognize abnormal findings.
basic life support (american heart assoc or red cross)
mn registered nurse (rn) license
