Clinical Case Manager - Day ShiftThe Clinical Case Manager is responsible for utilization reviews, resource management, discharge planning, treatment plan management, and financial management, while also completing medical record documentation. This role focuses on delivering superior quality care, treating others with respect, achieving objectives as a team, and overcoming obstacles resourcefully.ResponsibilitiesIdentify patient needs affecting treatment and discharge planning.Assist patients and families in utilizing hospital resources to adapt to healthcare situations.Collaborate with the interdisciplinary team to manage clinical plans and ensure safe discharges.Conduct thorough utilization reviews in accordance with regulatory standards.Maintain timely completion of medical documentation and reports.Ensure compliance with payor requirements and enhance guest relations.Maintain community resource systems to assist patients and families.Manage financial responsibilities associated with patient care and discharge plans.Required Experience / Certifications / LicensureCurrent licensure in a clinical discipline per state guidelines (RN, RT, SW preferred).Associate's Degree in a healthcare field (Bachelor's degree preferred).Working knowledge of the insurance industry and government reimbursement.Minimum of 2 years of experience in Case Management and Discharge Planning in an Acute Care or Rehab setting.ScheduleThis position is full-time with a Day shift schedule (5x8.0).Why ARMStaffing?At ARMStaffing, we take care of our employees!
