Responsibilities
Lead and supervise a team of registered nurses and healthcare professionals delivering mobile health services, ensuring adherence to clinical standards and organizational policies.
Coordinate patient assessments, medical documentation, and care planning tailored to individual needs across community-based settings.
Oversee clinical workflows including vital signs monitoring, medication administration, IV infusion, catheter care, specimen collection, and basic life support procedures.
Manage quality checks on electronic health record (EHR) systems such as Epic or Cerner to ensure accurate documentation and compliance with HIPAA regulations.
Facilitate discharge planning, family education, and health coaching to promote long-term wellness and disease management.
Ensure infection control protocols are strictly followed during all mobile visits; maintain sterile processing standards for medical equipment.
Collaborate with interdisciplinary teams on utilization review, case management, and treatment plans for complex cases involving dementia care, behavioral health, or pediatric patients.
Maintain up-to-date knowledge of ICD-10/ICD-9 coding for accurate medical records billing and reporting; support utilization management initiatives.
Requirements
Valid RN license with a Bachelor of Science in Nursing (BSN) from an accredited program.
Proven experience in critical care environments such as ICU or Level I Trauma Centers; hospital medicine or emergency medicine experience preferred.
Demonstrated expertise in diverse nursing settings including long-term care facilities, assisted living communities, home care, or pediatric units.
Strong leadership skills with prior supervising or nurse management experience; ability to motivate and develop team members.
Proficiency with EMR/EHR systems such as Epic, Cerner, or Athenahealth; familiarity with ICD coding (ICD-10/ICD-9).
Knowledge of physiology, anatomy, pharmacology, infection control practices, and medical terminology essential for comprehensive patient assessment.
Experience working with people with disabilities or developmental disabilities; familiarity with dementia or memory care is advantageous.
Ability to perform technical procedures including IV insertion, phlebotomy, catheterization, tube feeding management, ventilator support, and sterile technique.
Excellent communication skills for effective triage assessments and patient education; compassionate approach to senior care and behavioral health needs.
Current Basic Life Support (BLS) certification; additional certifications such as ACLS or PALS are a plus. Pay: $74,892.02 - $90,192.54 per year
