Position Details: Full-time Hospice RN Case Manager position Schedule: Monday thru Friday, 8a-5p Available territory openings: Hancock County and surrounding communites RN Case Manager typically assigned 1 county to cover. Job Functions: Performs physical and psychosocial assessment on all hospice patients. Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management. Responds to the physical, mental, social and emotional needs of terminally ill patients and their families. Assesses patient’s condition, initiates plan of care, re-evaluates and updates as necessary. Obtains all necessary physician orders and collaborates with physician as patient’s needs dictate.
