Good Shepherd helps patients and their families live each day fully, focusing on quality of life, control of symptoms and the things that matter to them the most.Job DetailsThe Community Intake Coordinator is responsible for receiving, processing and developing referrals, establishing and maintain professional relationships with key physician groups and referral sources, acting as a Liaison to referral sources on behalf of Good Shepherd Hospice, and identifying potential market growth opportunities and program expansion. Additionally, this position creates pre-admission evaluations based on medical records to present to Hospice Physicians to determine hospice eligibilityThe Successful Candidate Must Meet The Following Minimum RequirementsBachelor’s degree required. Prior Hospice, Palliative and/or Oncology strongly preferredExcellent organizational skills and attention to detail.
