CRITICAL CARE
ICU Model: Closed, 12-bed ICU (to include step down); admits by critical care intensivists 7 a.m. - 5 p.m.
APP admits overnight 6 p.m. - 6 a.m.
ICU Staffing Model: 10 hours per day in-house intensivist; 7 days per week
Mixed Medical-Surgical ICU
Respond to codes when on-site
Work collaboratively with the rest of the IAC team (ED, HM)

Hospitalists admit 85% of patients from the Emergency Department
Stroke patients who receive fibrinolytic or who meet criteria for arterial thrombectomy are transferred to San Antonio after consultation with a Tele-Neurologist
Specialty backup includes 24/7 cardiology (including cath lab and most cardiologists do PE thrombectomies), general surgery, ortho, and OB/GYN
Interventional radiology availabe during the week
Vascular surgery coverage averaging 20-25 days per month
Urology coverage 50% of the time

Staffing Model:
10-hour shifts
3 Physician shifts per day (total 30 hours)
3 APP shifts per day (total 30 hours)
Shifts will flex 1-2 hours up or down based on volumes
EMR: Meditech
BC (or BE) in Emergency Medicine (preferred) or BC in FP/IM with a minimum of five years of ED experience
Stroke certified; however, still transfer patients who require lytics and/or arterial thrombectomy
Primary Stroke Center certification expected by end of 2025
HOSPITALIST SERVICE
A cohesive team of 10 Hospitalist Physicians and 4 Hospitalist APPs
The team is supported by an administrative coordinator
Integrated Acute Care model where all providers are partners of one team covering Emergency Medicine, Hospital Medicine, and Critical Care Medicine
Most patients admitted through the ED come under the Hospitalist service
Hospitalists also provide medical consultation to the inpatient rehab unit staffed by dedicated physiatrists
Hospitalists hand off patients to the Intensivist when requiring transfer to the ICU

Average Daily Encounters: 14
Provides medical consultation services in coordination with dedicated physiatrists in the Acute Rehab Unit
Inpatient rehabilitation unit
Stroke Specialty program
Support of on-site hospitalist Physicians and APPs
The team is supported by an Administrative Coordinator
7 days on/7 days off schedule
Pre-operative clinic
Rehab APP provides clearance for elective orthopedic surgical candidates
Tuesday – Friday: 4 scheduled patients per day
