The ideal candidate is a self-starter who excels at one-on-one patient interaction and is dedicated to improving the lives of individuals recovering from illness or injury, managing chronic conditions, or needing assistance with daily living.Key Responsibilities:Patient Assessment & Care Planning: Conduct comprehensive initial and ongoing assessments of patient health, including physical, emotional, and psychosocial needs. Develop, implement, and update individualized plans of care in collaboration with physicians and other healthcare professionals.Clinical Care: Administer medications, including injections and IV infusions, and perform skilled nursing procedures such as wound care, catheter care, and ostomy care.Patient and Family Education: Educate patients and their families on disease management, proper medication administration, wound care techniques, and how to identify warning signs of complications. Empower them to become active participants in their own health and recovery.Monitoring & Documentation: Monitor and report changes in patient condition, vital signs, and progress. Maintain accurate and detailed electronic medical records in compliance with all state and federal regulations.Care Coordination: Act as the primary link between the patient, their family, physicians, and other members of the healthcare team (e.g., physical therapists, social workers, home health aides). Ensure a cohesive and holistic approach to care.Safety & Advocacy: Assess and address potential safety concerns within the patient's home environment. Advocate for the patient's needs and ensure they receive the best possible care.Emergency Response: Be prepared to respond to and manage medical emergencies in the home setting.Qualifications:Education: [Associate's Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN)]. A BSN is preferred.License: Active and unrestricted Registered Nurse (RN) license in NC.Experience: 2 years of recent clinical nursing experience. Experience in home health, hospice, med-surg, or acute care is highly preferred.Certifications: Current Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR) certification.Skills:Homecare Homebase Software Experience - RequiredOASIS Experience - RequiredAbility to work independently, manage time effectively, and prioritize a caseload of patients.Valid driver's license and reliable transportation.
