The ideal candidate will deliver safe, high-quality care while collaborating with interdisciplinary care teams and maintaining compliance with facility policies and professional standards.ResponsibilitiesCoordinate and plan patient discharge and care transitions in an acute care setting. Assess patient needs and develop individualized care plans. Collaborate with multidisciplinary teams to ensure patient care goals are met. Maintain accurate and timely charting using EPIC systems. Ensure compliance with CMS regulations and hospital policies. Float between various units, including Colby and Pacific campuses as needed.QualificationsCurrent RN licensure or equivalent in the state of Washington. 1 year of acute care experience required; case management experience preferred. Discharge planning and care coordination experience required. Basic Life Support (BLS) certification required. Experience with EPIC charting system highly preferred.
