These aren't just words — they're how we work every day.The RoleAs a Case Manager, you'll be the connective tissue between our clients, their families, and the full spectrum of care they need. From intake through discharge, you'll ensure every child on your caseload receives individualized, coordinated, and compassionate support.Schedule: Monday – Friday, 8:00 AM – 5:00 PM (some slight scheduling flexibility is required for specific cases)What You'll DoIntake, Assessment & Treatment PlanningAdminister CALOCUS assessments to determine appropriate levels of careComplete intake paperwork and conduct comprehensive mental health needs assessmentsDevelop individualized service, safety, and treatment plans for each clientUpdate treatment plans, risk assessments, and clinical documentation every six months or as neededDevelop and implement transition, discharge, and aftercare plans before services endService CoordinationCoordinate supportive services for clients and familiesCommunicate weekly with guardians and caregivers on treatment progressConduct at least one monthly in-person or virtual check-in with every family on your caseloadParticipate in departmental staffing meetings and debrief with the BHT teamClient AdvocacyAdvocate for client needs and ensure access to appropriate servicesAssist families with tasks requiring extra guidance, such as AHCCCS appointments and crisis navigationFacilitation & Community EngagementFacilitate Child and Family Team (CFT) meetingsAttend quarterly school staff meetings to provide client progress updatesBuild and maintain relationships with community resources and referral partnersDocumentation & ComplianceMaintain accurate, up-to-date client records including case notes, service plans, and monthly progress reports in compliance with state regulations and company policyCrisis InterventionProvide crisis intervention and support to clients and families as neededWhat You BringSpanish language proficiency strongly preferredStrong organizational, planning, and task management skillsAbility to maintain composure under pressure and adapt quicklyEmpathy and skill in building relationships with families from diverse backgroundsClear verbal and written communication with clients, families, and clinical professionalsCritical thinking and problem-solving abilityComfort with technology, including Google Workspace and Adobe PDFAbility to work both independently and collaboratively as part of a teamEducation & ExperienceBachelor's degree in social work, psychology, or a related field preferred; relevant experience may substituteMinimum of one year of experience (including internships or volunteer work) with children with mental health needsCompensation & BenefitsAnnual Salary: $40,000 – $45,000 per yearHealth insuranceDental insuranceVision insuranceLife insurance401(k)Paid time off
