Therapy/Rehabilitation/PT Home Health: Duration:13 Weeks Start:01/05/2026 Shift: Day 5x8-Hour (08:00 - 17:00) Description: Home Health Physical Therapist delivers rehabilitative therapy to patients in their homes to improve mobility, strength, and functional independence. They perform detailed assessments, develop personalized treatment plans, and provide therapeutic exercises and interventions. The PT tracks progress, adjusts therapy as needed, and ensures accurate clinical documentation. They also educate patients and caregivers on safe mobility techniques, equipment use, and long-term health maintenance. Req Certs: State License BLS Responsible for assessing, planning, organizing, and participating in rehabilitative programs that improve patient mobility, relieve pain, increase strength, and improve or correct disabling conditions resulting from disease or injury. Gains confidence and cooperation from the patient, their family/support group, and other healthcare providers through competent patient assessment, attentive monitoring and care, and effective communication. Adheres to all local/state/federal regulations, codes, policies and procedures to ensure privacy and safety while delivering optimal patient care. Provides clinical supervision of assistants and aides, and serves as clinical instructor for assigned students.
