Job Title: Social WorkersEmployment Type: ContractWork Hours: 37.5 hrs./weekWork site: 100% OnsiteLocation: 4 Harriman Drive, Goshen, NY 10924Job Duration: 2 YearsClosing: 04/06/2026.Minimum Education & Experience:Master's Degree required.Licensed Master Social Worker1-2 years of minimum experience.Duties, include but at not limited to:Maintain a caseload of 8 children in the adolescent IDT program, 6th – 12th gradeParticipate in IDT admissions with child, family, MD, and school liaisonProvide Weekly Individual TherapyProvide Daily Group TherapyProvide Family Therapy and Crisis Intervention as neededFacilitate Transition Meetings for each student on their caseload that include the student and family, IDT teacher, school liaison and other professionals from the student's home districtCollateral involvement with participating schools and agenciesFunction as part of an interdisciplinary treatment teamParticipate in weekly Treatment Team meetingsAdvocate for youth and families to obtain community-based linkages with additional supportive services that meet their needsMaintain timely medical record documentationFacilitate thorough discharge planning and complete discharge follow-upCollaborate with school liaisons throughout Orange County to facilitate the referral and screening processMisc.
