Homecare RN Case Manager (Mon-Fri, 8am-5pm)

Full time, salaried, exempt positionWork Schedule: Monday-Friday, 8am-5pmService area: within a 30-mile radius of our High Point, NC officeThis position demonstrates competency in delivery of quality patient care to patients ranging in age from infants to geriatrics. The RN Case Manager is responsible for developing and implementing a plan of care in accordance with the patient's expressed wishes and delegates care and interventions in full compliance with the Nurse Practice Act and organizational policy, procedures and standards of care.What are the essential duties day to day?In charge of scheduling patientsChecking vitalsMedicine reconciliationDocumentation for each visit (assessments, interventions, and updates)Develop and coordinate plan of careCommunicate with Nurse PractitionerAverage of 4 patient visits a daySupervise CNA services provided to assigned patientsMake calls to family to explain what you see regarding patient changesServe as an active member of the interdisciplinary group (IDG) and attend IDG meetings regularlyHere is what employees in this role have to say!I love the autonomy of this job.
