PRN Physical Therapist for Home Health Visits (Redwood City, CA)

Job Description OverviewActive In-Home Therapy is looking for a skilled and dedicated Physical Therapist to join our team. This is a PRN position (not full-time), meaning you'll work with us on an as-needed basis while enjoying flexible scheduling.You'll be hired as a W-2 employee, not as an independent contractor.This role is an excellent option if you're looking to supplement your current work while making a meaningful difference in patients' lives.What You'll DoConduct thorough assessments of patients' physical conditions to determine rehabilitation needs.Develop and implement individualized treatment plans in collaboration with the healthcare team.Provide hands-on physical therapy interventions to improve strength, flexibility, balance, and coordination.Educate patients and their families on therapeutic exercises, mobility aids, and safety strategies to promote independence.Monitor and document patients' progress, adjusting plans as needed.Communicate effectively with patients, families, and team members to ensure a positive and supportive care experience.What We're Looking ForBachelor's or Master's degree in Physical Therapy from an accredited program.Current California Physical Therapist license.Home health experience preferred but not required.Strong clinical judgment, organization, and communication skills.Reliable transportation for patient visits.
