Job Title: Home Health Physical Therapist Reports to: Clinical Manager of Therapy Pay: Up to $48/hour DOE. Hours: Monday-Friday 8am-5pm. We are looking to immediately hire a qualified Home Health Physical Therapist. We have multiple territories throughout our 60 mile coverage area in the Willamette Valley. Your assigned territory will be discussed based on patient need and your preferred location. Each PT sees approx. 5 patients a day with an ongoing caseload of approximately 25 patients. Home Health Physical Therapist Duties & Responsibilities: Provides rehabilitative and skilled PT services to patients in their place of residence. Performs manual and functional therapy exercises and activities with the patient. Makes the initial evaluation visit to patients and develops/implements an appropriate Plan of Care Advises and consults with families/caregivers and the multi-disciplinary team Assists in the supervision of PTA's and CNA's Uses various tests to determine patient's individualized needs, and revises the plan of care as needed. Completes Point of Care Documentation Meet agency productivity expectations Job requirements: Graduate of accredited Physical Therapy education program.
