Job Title: Corporate Director of Case ManagementLocation: Nashville, TNEmployment Type: Full-time, No WeekendsSalary Range: $$,400 min - $$,700 max - Based on years of experience.Bonus: Up to 15% annual bonus, based on performance KPIs.Relocation Assistance: Case by case.Job Summary and Qualifications:We are currently seeking a highly qualified and motivated individual for the role of Corporate Director of Case Management. This key position is responsible for assessing, planning, coordinating, developing, and directing Case Management/Transitional Care, Post-Acute Services support, Social Work programs, and Case Management related Regulatory Standards. The Director will also oversee initiatives in areas such as automation and information systems.Expected Job Duties:Continuously evaluate facility practices and industry changes related to Case Management.Conduct needs analyses, identify gaps, and develop program components.Develop and implement evidence-based toolkits to support Case Management initiatives.Collaborate with cross-functional teams to implement programs that achieve strategic objectives.Ensure compliance with governmental and regulatory agencies in case management and discharge planning policies and procedures.Provide feedback to Case Management Leadership and other clinical areas through the approved data reporting process.Education/Experience Requirements:Bachelor's in Nursing required or licensed social worker (LCSW).Master's degree preferred.5+ years of Case Management experience required.3+ years of Case Management leadership experience required.Performance Improvement experience required.Strong oral, written, and presentation skills.Exp. with Discharge planning in an acute care setting.Certification/Licensure Requirements:TN RN or LCSW State License.Basic Cardiac Life Support (BLS).Shifts: Days, no direct reports.
