Key Responsibilities for Case Manager RN: Coordinate and oversee patient care plans to ensure continuity and quality of care. Perform patient assessments, including psychosocial, medical, and functional evaluations. Collaborate with interdisciplinary teams to develop individualized care plans and monitor patient progress. Act as a liaison between patients, families, and healthcare providers, facilitating effective communication and resource coordination. Advocate for patient needs while ensuring compliance with healthcare regulations and standards. Maintain accurate documentation of patient care activities in accordance with facility policies and regulatory requirements. Educate patients and families on care plans, available resources, and post-care expectations. Case Manager RN Qualifications: Active and unrestricted New Mexico Registered Nurse (RN) license. Graduation from an accredited nursing program required. BLS certification required. At least 2 years of clinical nursing experience preferred; prior case management experience is a plus. Strong interpersonal and communication skills, with a patient-centered approach. Ability to work collaboratively in a team environment and independently manage a diverse caseload.
