We are currently interviewing for a PRN (transition to Full-Time upon eligibility & availability) LVN Field Nurse to cover the Kaufman, Scurry, Crandall, Terrell and surrounding areas.We are looking for a Home Health LVN to provide skilled nursing care to patients in their homes under the direction and supervision of a Registered Nurse. This role supports the delivery of safe, high-quality patient care through routine nursing visits, treatment administration, patient education, and timely documentation. Previous Home Health Care experience is a must. Familiarity with electronic charting (KINNSER/AXXESS) is a plus.Responsibilities:Provide skilled nursing care to home health patients under the supervision of an RNPerform routine nursing visits including vital signs, medication administration, injections, and treatmentsAssist with wound care and dressing changes per physician ordersMonitor patient condition and promptly report changes to the supervising RNEducate patients and caregivers on medications, treatments, and safetyAccurately document all visits and care provided in the agency EMREnsure compliance with Medicare, Texas regulations, and agency policiesCoordinate visits and communicate effectively with the clinical teamQualifications1. Must currently be licensed in the state of Texas2. Should have at least two years clinical experience3. Prior Geriatric home health care experience is a MUST4. Should have Reliable Transportation & Car insurance5. Must be able to pass background check & have valid CPR & TB test/Chest x-ray6. Bilingual is a PLUS8. Authorized to legally work in the United States7.
