Chronic Disease Nurse (Diabetes/Hypertension)

POSITION SUMMARY: Perform the duties required to coordinate and monitor nursing services for the Chronic Disease Department. Provide direct care with patients, work with Health Coaches and BCCCP Follow-Up Assistant.POSITION QUALIFICATION REQUIREMENTSEducation: Bachelor of Science Degree in Nursing.Experience:* Five (5) years' experience in ambulatory care, hospital and /or community health nursing.* Experience in ambulatory care, hospital or community health nursing.Skills and Abilities which may be representative but not all inclusive of those commonly associated with this position:Good organizational, analytical, and problem-solving skills.Excellent verbal and written communication skills.Able to work with people with diverse ethnic backgrounds.Strong computer skills including the use of word process, spreadsheet and e-mail software.Able to work on multiple tasks simultaneously.Residency and Transportation Requirement: Must possess a valid driver's license and automobile with insurance coverage as required by the State of Missouri.License(s)/Certification(s) Required: Licensed RN in Missouri and CPR certified.FUNCTIONS OF THE JOBEssential FunctionsWork with health coaches' compliance to established policy and procedures with daily work schedules.Recruit, patients with Chronic Disease conditions and work with qualified health coaches and BCCCP Follow-up Assistant.Coordinate and implement training, educational classes and in-services for staff in chronic disease management, patients and JACHO standards.Assist in the development and implementation of policies and procedures for Chronic Disease department and other areas as required.Coordinate and oversee special programs as assigned.Prepare and maintain needed records and reports.Prepare and revise educational materials for clients with chronic diseases.Complete outreach/in reach and follow up activities provided through BCCCP.Participate as a member of the Health Disparities Collaborative teams dealing with chronic diseases.Work in coordination with health center nurse coordinators and supervisors to monitor and maintain adherence to JCAHO and other accrediting body standards.Represent Affinia Healthcare in the community and make presentations as requested.Monitor the goal achievement of Healthy Heart Program and other grants and contracts involving coaching of clients with chronic diseases.Participate in the planning and evaluation of new and existing programs.Perform other duties as assigned.Marginal Functions: Orientates volunteers, physicians, nurse instructors and students.Supervises: NoneNOTE: SMOKING IS PROHIBITED IN THE WORK ENVIRONMENTNOTE: ALL APPLICANTS MUST PROVIDE CONTACT INFORMATION FOR THREE REFERENCESAN EQUAL OPPORTUNITY SERVICES PROVIDED ON A NON-DISCRIMINATORY BASIS
