Job Description ACCENT GROUP AND FAMILY SERVICESJOB DESCRIPTIONJOB TITLE: Intensive In- Home Counselor (QMHP-C or T Registration Required- Resident PREFERRED)IMMEDIATE SUPERVISOR: Director of ServicesDESCRIPTION: Counselors are responsible for a variety of counseling and educational services within the home and community setting. Counselors are also required to develop, coordinate, and implement the treatment goals and objectives of each client, to include ensuring all objectives, goals, and deficiencies are documented and reported accurately and in compliance with organizational policies and guidelines. Counselors will facilitate clients support groups as needed as well as build partnerships with other community organizations.TASKS:1. Participates in initial client assessment and ongoing reports following the approved format2. Participates in the service plan3. Has weekly contact with the Referring Agency4. Arranges for weekly case supervision with clinical supervisor5. Acts as a co-leader of parenting and peer education/support groups as needed6. Acts as the liaison between family, referring agencies, and other community organizations.7. Coordinates client services between various agencies8. Maintains case records and documents services rendered9. Attends weekly staff meetings consistently. Prior approval from management team is required when absent.10. Coordinates and facilitates staffing for the client: inviting the family of the client, the Referral Source, representation from other pertinent agencies providing services and supervision prior to the end of each thirty-day service period11. Maintains high degree of client confidentiality12. Works with other staff assigned to cases, communicating effectively13. Maintains a high degree of professionalism in the community with clients and with referring agencies seeking to build and maintain positive relationships14. Assists in providing individual and family counseling services to clients in the home and community setting15. Must successfully pass a criminal history check by the BIU of DBHDS and a central registry check by VDSS. QMHP-C or T Registration Required with the VA Board of Counseling.
