Family Direct Support Professional Job DescriptionJob Overview:This position is responsible for providing individualized support, skills training, positive direction, and assistance to individuals who experience a developmental disability in accordance with state and program policies and procedures and their own personal outcome and goals.Outcome:To assist and coach an individual to achieve their goals and desired outcomes and to live successfully independently.Employment Requirements:Minimum of 20 hours a weekMust be 18 years of age, or olderHigh school graduate or equivalentAdditional experience with people who experience developmental disabilities preferredMust possess a valid driver's license, access to a reliable vehicle, current auto insurance, and good driving recordClear background checks as defined by state regulations and policiesAbility to do daily standing, walking, bending, and lifting (up to 20 lbs)Must possess good verbal, reading, and written communication in English; basic math skills; and basic computer skillsAbility and willingness to successfully complete and apply trainingInterest in assisting people supported to achieve personal outcomes and to provide person centered supportsAbility to work effectively and harmoniously with others; be of good moral character; have an interest in the welfare of the individuals; have a willingness/ability to communicate with and relate to individuals, parents, guardians, fellow-staff, representatives of community agencies, and company managementFlexibility with respect to hours of work and changing conditionsAdditional certifications may be required by state regulationsExhibits a talent for:Inclusion: Ability to accept others where they are at, as they are, and actively include them for the purpose of helping them engage in life to its fullestRelating: Enjoy getting to know others and working with them to achieve a common goalCommunication: Ability to consistently and reliably communicate with all members of their team in an effective mannerResponsibility: Exhibits the values of honesty and integrity by following through with their word Responsibilities: Provides supports to individuals by assisting in the development and implementation of all support plans, including assisting people in achieving their personal goals and desired outcomes as outlined in their service agreement and provide interaction and choices for activities that support their goals, and supporting people to be independent in activities of daily livingAssists with information gathering for Personal Outcome Measures, and other assessmentsAssists people with maintaining a clean, safe, and orderly home by following safety and infection control procedures; educates people about maintaining homesAssists people with getting to appointments, errands, and to various locations in the community by driving them to and from said locationsMaintain and expand skills and knowledge relevant to providing high quality support and servicesObtain and maintain all required certifications and trainings and apply the information to support people servedMaintain open communication and cooperative work effort with management, staff, customers, and other support staff to assure quality and continuity of support to people servedSupports the ACC Mission and Core Values while empowering each person to direct their own life choicesCompletes proper and timely documentation including daily logs, incident reports, progress notes, etc. in a manner that meets or exceeds the requirements of ACC, brokerages, and/or state regulations while with the customerAssists people with fiscal management while safeguarding their funds. This may include assistance in balancing checkbooks, paying bills, or budgetingEnsures that people exercise their rights and that rights are not restricted without due process, ensuring individuals are treated with dignity and respectPerforms other duties as assigned
