Greetings,Hot Need Mobile Physician to do Adult-Geri in Central/Western, CT and Westchester County/Bronx, NY Certified to see Medicare and Medicaid patients, ISNP or SNF experience preferred, reliable mode of transportation. HighlightsMon-Fri; 8AM-5PM. This is strictly a travel role.Provider will be covering central and western CT(Harford, Middletown, Farmington, Bloomfield, New Britain, Danbury, Bridgeport, Milford, etc) and Westchester County/Bronx area of NY(must be dual licensed).Visiting SNF and ISNP facilities in these areas as well as virtual visits.Geriatrics, consider "advanced" primary care as these patients are dealing with multiple medical issues.Prefer candidates with background in Geriatrics, SNF, or ISNP.Would also prefer that the provider is based in the Central/Western area of CT.
