Summary We are seeking a skilled Physical Therapist and Physical Therapists Assistants to join our team. In this role, you will be responsible for developing and implementing personalized therapy plans that enhance the physical well-being of our patients. Your expertise will play a vital role in helping individuals regain their independence and improve their quality of life. Responsibilities Evaluate patients' physical abilities and limitations to develop effective treatment plans. Implement individualized physical therapy programs based on patient needs and goals. Monitor patient progress and adjust treatment plans as necessary to ensure optimal outcomes. Collaborate with other healthcare professionals to coordinate comprehensive care for patients. Educate patients and their families on exercises, techniques, and lifestyle modifications to support recovery. Maintain accurate documentation of patient assessments, treatments, and progress reports in EMR - Wellsky. Requirements Valid state license to practice as a Physical Therapist or Physical Therapist Assistant Strong knowledge of anatomy, physiology, and physical therapy techniques. Experience in developing care plans tailored to individual patient needs. Familiarity with geriatrics and orthopedics is preferred. Excellent communication skills for effective interaction with patients and families.
